Notice of Privacy Practices (Remote Physiologic Monitoring)
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
This Notice explains how MedStreamline LLC uses and protects health information created or maintained as part of Remote Physiologic Monitoring (RPM). RPM is provided as a limited, supportive service and does not replace your primary care provider or facility‑based healthcare providers.
Your Rights
With respect to the health information we create or maintain as part of RPM, you have the right to:
- Get a copy of your RPM‑related paper or electronic health records
- Ask us to correct RPM‑related records you believe are incorrect or incomplete
- Request confidential communications related to RPM (for example, asking us to contact you in a specific way or at a specific location)
- Ask us to limit how we use or share your RPM‑related information
- Get a list (accounting) of certain disclosures of your RPM‑related information
- Get a copy of this Notice of Privacy Practices
- Choose someone to act for you, such as a medical power of attorney or legal guardian
- File a complaint if you believe your privacy rights have been violated
Your Choices
You have choices about how we use and share RPM‑related health information in certain situations.
You may request that we share RPM-related health information with individuals you identify, such as a family member, caregiver, personal representative, who are involved in your care or payment for your care.
We otherwise use and disclose RPM‑related information as permitted or required by law, including for treatment, payment, healthcare operations, and to provide RPM.
We do not sell your health information.
Our Uses and Disclosures
We may use and share health information we create or maintain without your written authorization to support RPM and to comply with applicable law. For example, we may use and share your information as we:
- Provide and manage RPM
- Review and monitor physiologic data transmitted from approved medical devices
- Communicate RPM‑related information with your facility care team and treating providers to support your ongoing care
- Operate our RPM program, including quality improvement, training, and administrative activities
- Bill for RPM, when applicable
- Support public health and safety activities as required or permitted by law
- Do research, when permitted by law and subject to applicable protections
- Comply with federal, state, and other legal requirements
- Respond to lawful requests from government authorities, including workers’ compensation, law enforcement, and regulatory oversight
- Respond to lawsuits and legal actions
Your Rights
When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.
These rights apply to the health information we create or maintain as part of our RPM.
Get a copy of your RPM-related health information
You may request an electronic or paper copy of the health information we maintain about you in connection with RPM.
We will provide a copy or a summary of your RPM‑related health information, usually within 30 days of your request. In certain cases, we may extend this period by up to an additional 30 days if we notify you in writing of the reason for the delay. We may charge a reasonable, cost‑based fee.
Ask us to correct your medical record
You may request that we amend RPM‑related health information you believe is incorrect or incomplete. Ask us how to submit a request.
Certain information, such as device‑generated data, timestamps, or system‑captured measurements, may not be subject to amendment because they accurately reflect information as recorded. We may deny your request, but we will tell you why in writing, usually within 60 days. In certain cases, we may extend this period by up to an additional 30 days if we notify you in writing of the reason for the delay.
Request confidential communications
You may request that we communicate with you about your RPM‑related health information in a specific way or at a specific location (for example, by phone, mail, or another method you identify).
We will accommodate reasonable requests for confidential communications while continuing to communicate with you as needed to provide RPM.
Ask us to limit what we use or share
You may request that we limit how we use or disclose certain RPM‑related health information for treatment, payment, or healthcare operations. We are not required to agree to your request, and we may deny it if it would affect your care or our ability to provide RPM.
If we agree to your request, we may still use or disclose information as needed for emergency situations or as otherwise required by law.
If you pay for RPM out‑of‑pocket in full, you may request that we not share that information for payment or healthcare operations with your health insurer. We will honor that request unless a law requires us to share the information.
Get a list of those with whom we’ve shared information
You may request a list (an accounting) of certain disclosures of your RPM‑related health information that we have made during the six years prior to the date of your request, including who the information was shared with and why.
This accounting will not include disclosures made for treatment, payment, or health care operations, or other disclosures that HIPAA does not require to be included. We will provide one accounting in a 12‑month period at no cost and may charge a reasonable, cost‑based fee for additional requests.
Get a copy of this Notice of Privacy Practices
You may request a paper copy of this Notice of Privacy Practices at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.
Choose someone to act for you
If someone has authority to act for you as your personal representative–such as a person with medical power of attorney, a legal guardian, or another individual authorized under applicable law–that person may exercise your rights and make choices about your RPM‑related health information.
We may rely on a good‑faith belief that a person has authority to act for you, based on information provided by you, your facility, or others involved in your care, unless we have reason to believe the representation is inaccurate.
File a complaint if you feel your rights are violated
You may file a complaint if you believe we have violated your privacy rights by contacting us using the information listed in this Notice.
You can file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting https://www.hhs.gov/hipaa/filing-a-complaint/index.html.
We will not retaliate against you for filing a complaint.
Your Choices
For certain health information, you can tell us your preferences about how we share information. If you have a preference in the situation described below, please let us know, and we will follow your instructions as permitted by law.
You may request that we share RPM‑related health information with individuals you identify, such as family members or others involved in your care or payment for your care
We otherwise use and disclose RPM‑related health information with you, your authorized representatives, and your facility care team and treating providers as necessary to provide RPM and as permitted or required by law.
We do not sell your health information.
Our Uses and Disclosures
How do we typically use or share your health information?
We typically use or share your health information in the following ways to support RPM and to meet legal requirements.
Provide remote physiologic monitoring (RPM)
We may use your health information and share it with your facility or treating providers to support RPM and your ongoing care.
Example: We review vital sign data transmitted from approved medical devices and share relevant RPM findings with your treating provider or facility care team
Run our organization
We may use and share your health information to operate our RPM program, improve service quality, and contact you when necessary regarding RPM.
Example: We use RPM‑related information to monitor service performance, support quality improvement activities, and coordinate RPM communications.
Bill for your services
We may use and share your health information to bill and receive payment for RPM when applicable.
Example: We share necessary information with a health plan or other payer so that RPM can be billed and paid, as permitted or required by law.
How else can we use or share your health information?
We are allowed or required to share your health information in other ways — usually in ways that contribute to the public good or comply with legal requirements. We must meet specific conditions under the law before we can share your information for these purposes.
Help with public health and safety issues
We may share RPM‑related health information about you for certain public health and safety activities, as permitted or required by law, such as:
- Preventing or controlling disease, injury, or disability,
- Reporting adverse events or device‑related issues
- Reporting suspected abuse, neglect, or domestic violence
- Preventing or reducing a serious and imminent threat to anyone’s health or safety
Do research
We may use or share your health information for health research purposes, when permitted by law and subject to applicable privacy protections.
Comply with the law
We will share health information about you when required to do so by federal, state, or local law. This includes sharing information with the U.S. Department of Health and Human Services if it requests information to confirm our compliance with federal privacy laws.
Address workers’ compensation, law enforcement, and other government requests
We can use or share health information about you:
- For workers’ compensation claims or similar programs
- For law enforcement purposes or with a law enforcement official, as required or permitted by law
- With health oversight agencies for activities authorized by law, such as audits, investigations, or inspections
Respond to lawsuits and legal actions
We may share health information about you in response to a court or administrative order, or in response to a subpoena or similar legal process, as permitted or required by law.
These disclosures relate only to health information we create or maintain in connection with our RPM.
Our Responsibilities
We are required by law to maintain the privacy and security of your protected health information that we create or maintain in connection with RPM.
- We will notify you without unreasonable delay, and in no case later than as required by applicable law, if a breach occurs that may have compromised the privacy or security of your RPM‑related information.
- We must follow the duties and privacy practices described in this Notice and provide you with a copy of it.
- We will not use or share your health information other than as described in this Notice unless you authorize us to do so in writing. If you authorize us to use or share your information, you may revoke that authorization at any time by notifying us in writing. Your revocation will not affect any uses or disclosures we made in reliance on your authorization before we received your written revocation.

Online Tracking Technologies and Consent
Our website and online services may use tracking technologies such as cookies, web pixels, software development kits (SDKs), session replay tools, chatbots, and analytics services, some of which are provided by third parties. These technologies may collect information about your interactions with our online services, including device and browser information and information you enter into online forms. To the extent information collected through these technologies constitutes protected health information, it will be handled in accordance with this Notice.
Where required by applicable law, we will obtain your affirmative consent before these technologies collect or transmit your information. We will make this Notice and our online Terms of Service clear and readily accessible to you at the time your consent is requested.
We configure these technologies to mask, redact, encrypt, or otherwise suppress sensitive information—such as health information, payment card numbers, and similar identifiers—using commercially reasonable measures designed to reduce the likelihood that such information is captured or transmitted in readable form.
We contractually require our technology vendors to act solely as our service providers. Our vendor agreements prohibit vendors from aggregating, selling, or using your information for their own independent purposes, including to train artificial intelligence models or to improve their own products and services, except as permitted by applicable law and our agreements..
Children’s Privacy
Our website is not intended for children under the age of 13, and we do not knowingly collect personal information from children. If you are a parent or guardian and believe that a child under the age of 13 has provided information to us through our online services, please contact us using the information in this Notice, and we will take reasonable steps to delete such information. This provision does not affect health information we create or maintain in connection with RPM provided to a patient through that patient’s personal representative as described in this Notice.
Changes to the Terms of this Notice
We may change the terms of this Notice from time to time, and any revised Notice will include an updated effective date. Any changes will apply to all RPM‑related health information that we maintain about you as of the new effective date. The updated Notice will be made available upon request, will be posted on our website, and will be distributed as required by applicable law.
